This role is responsible for accurately dispensing medications to patients, ensuring the availability of necessary drugs, and processing correct billing in accordance with established procedures and the hospital’s patient care standards.
ROLES AND RESPONSIBILITIES
Deliver a superior pharmacy service to patients by collaborating seamlessly with other healthcare professionals within assigned areas.
Verify prescriptions to confirm accuracy and appropriateness, and proactively collaborate with prescribing physicians to address any discrepancies, thereby promoting the safe, cost-effective, and rational utilization of medications and other pharmaceutical products.
Observe and evaluate patient reactions to administered treatments, potential drug interactions, and disease-related medication effects, ensuring prescribed medications align with specific indications.
Record any undesirable or harmful reactions patients may have two prescribed medications and promptly report these incidents for further evaluation and action.
Perform ongoing clinical assessments of treatment sheets and prescriptions to verify the appropriateness of treatments, identify potential drug interactions, confirm dosing accuracy, and ensure all prescribed doses are administered correctly.
Engage collaboratively in the drug inventory management cycle, which includes initiating stock requisitions (ordering), receiving deliveries from central stores and ensuring timely placement on shelves, conducting regular stock audits (daily, random, and quarterly physical counts), and reconciling inventory discrepancies to maintain accurate records.
Handle all pharmaceutical-related matters that demand immediate action, including managing drug pricing, monitoring patient credit balances, and coordinating the replacement of compromised or outdated inventory.
Verify all pharmacy medications for upcoming expiration dates and promptly return any drugs nearing expiry to the store, strictly adhering to the Short Expiry Drug Policy guidelines.
Ensure accurate preparation and proper labeling of extemporaneous formulations for both inpatient and outpatient settings, as well as solutions for ward use.
Assist in developing and executing hospital policies pertaining to pharmaceutical practices and pharmacy standard operating procedures.
Engage in ward pharmacy services in accordance with hospital-established policies and procedures.
In strict compliance with the cytotoxic handling policy, assist with the preparation and reconstitution of chemotherapy medications.
Offer comprehensive patient counseling services and distribute informative leaflets to ensure all in-patients and outpatients receive necessary information whenever needed.
Maintain ongoing communication with the Pharmaceutical Society of Kenya, the Ministry of Health, and other relevant professional organizations to ensure the department remains informed of all advancements in the pharmaceutical sector.
Deliver comprehensive on-the-job training to newly hired staff and pharmacy technicians, ensuring they acquire essential skills and knowledge to perform their roles effectively.
