Ward Case Managers: CHSI/WCM/01/2022 (2 Positions)
Overall Job Function
To oversee the provision of Determined, Resilient, Empowered, AIDS-free, Mentored and Safe (DREAMS) activities and services to eligible Adolescents, Girls, and Young Women (AGYW) enrolled under the DREAMS Program within the Ward. This includes providing technical support to the project implementation staff in project design, implementation, monitoring and evaluation, program quality and effectiveness, effective documentation, and reporting among others.
Key Responsibilities
Coordinate with various stakeholders and Sub County Health Management Team to ensure smooth implementation of the project work plan and budget
Participate in enumeration/ screening, and enrolment of the eligible AGYW into the DREAMS Program
Participate in the identification, engagement and training of mentors to provide services to the AGYW
Liaise with community members and stakeholders on service provision to DREAMS Program beneficiaries
Provide support supervision to DREAMS field staff in the respective wards, including mentors to ensure quality service provision and achievement of layering for the AGYWs
Oversee all transition, graduation and exit processes of AGYW within the wards
Commission of new safe spaces and follow up of the established ones to ensure smooth and efficient running
Oversee the provision of adequate linkage and referral of AGYW for DREAMS’ services and services not offered by the DREAMS Program
Liaise with the committees for fair selection of AGYW into education subsidy and vocational training
Prepare and submit quality progress reports on programs’ activities on a monthly basis and as required to the technical advisor
Perform any other duty assigned by the immediate supervisor.
Person Specification
Diploma or its equivalent in Social Sciences, Education or related field. A degree is an added advantage
3 years experience in a similar field or position
Good report writing skills
Results orientated
Good leadership skills
Relevant computer skills.
