Program Coordinator – Community-Based NCD Program

Position Overview: We are seeking a highly motivated and experienced Program Coordinator to design and implement our Community-Based NCD Program. The successful candidate will be responsible for developing, managing, and evaluating all aspects of the program, ensuring it meets its objectives and improves health outcomes in our catchment community.
Key Responsibilities
Program Design and Development

Design training materials and protocols for Community Health Promoters (CHPs) on NCD screening, data collection, and referral processes.
Establish partnerships with public health officers, pharmaceutical companies, communities, and other stakeholders.

Program Implementation

Oversee the deployment of CHPs for community outreach, screening, and referral activities.
Ensure the provision of physician consultations, lab tests and NCD medications through strategic partnerships.

Technology Integration

Oversee the deployment and utilization of the mobile application for NCD screening, patient referrals, and management.
Implement AI-enhanced self-screening tools to facilitate early detection and self-monitoring of NCDs by community members.

Monitoring and Evaluation

Conduct regular data collection and analysis to track patient outcomes and program effectiveness.
Prepare quarterly and annual reports for hospital management and stakeholders.

Community Engagement

Lead community outreach efforts to promote the NCD program and educate the public on the importance of early detection and management.
Address any challenges or barriers to program implementation and community acceptance.

Budget and Resource Management

Develop and manage the program budget, ensuring efficient use of resources.
Identify and secure additional funding and resources to support program sustainability and expansion.
