Concept, Design and Development of a Youth Friendly Health Facility in Transmara

Background
Kakenya’s Dream is a not for profit organisation which invests in girls from rural communities through education, health and leadership initiatives. Our programs operate in Transmara, one of the remotest and most marginalized regions of Kenya, where nearly 80% of girls continue to undergo female genital mutilation (FGM) at puberty, 50% are married before the age of 19, and only 17% complete primary education. Girls’ education is not a priority for family or community investment, and social norms perpetuate low educational achievement. Investing in girls’ education yields profoundly positive outcomes not only for the girl, but for her family and community as well.
Our interconnected programs provide educational opportunities for vulnerable girls and train youth on sexual and reproductive health and rights in marginalized communities.

Centers for Excellence: Our boarding schools, the Kakenya Centers for Excellence (KCE I and KCE II), serve 280 girls in grades 4-11 in a safe, supportive environment. The program takes a holistic approach to educating girls, incorporating health and leadership elements into the curriculum and providing for all physical and social-emotional needs.
Network for Excellence: We support the 135 girls who have graduated from our boarding schools as they continue on their educational journeys. The girls receive mentorship, tutoring, financial support, and a variety of essential resources to help them thrive in secondary and tertiary education. This year, 83 Kakenya’s Dream girls are attending colleges in Kenya and abroad.
Health and Leadership Training: In the communities we serve, sexual and reproductive health are not discussed openly. Young people do not know their rights and cannot advocate for themselves. Our training program reaches boys and girls in schools across the region to foster positive change. So far, we have trained 14,000 youth over the past eight year.Kakenya’s Dream intends to set up a youth-friendly health facility to provide basic healthcare services including stigma-free sexual and reproductive health services, to its students and, eventually, to all youth, including boys, aged 10 to 24 years from the surrounding community.

Purpose of the assignment
The health clinic consultant will lead the conceptualization, designing and development of a functional youth-friendly health facility to benefit both the students & beneficiaries of Kakenya’s Dream and adolescent youth from the surrounding community.
Key areas of responsibility and deliverables

Develop the concept of a youth-friendly health facility, to include:
Detailed task list, timeline, and key party responsible for all aspects mentioned below (Gantt chart).

Medical components

Review and document country standards for quality healthcare services for Adolescents and youth for guidance on the required quality of services.
Seek all the required approvals for the set up and operation of a health clinic.
Develop a 5-year human resources framework including functional job descriptions for all the health clinic staff and guide in the hiring process of the clinic staff.
Develop a list of all the recommended supplies and equipment required in the clinic, including a computer system to track medical records.

Programmatic components to entice youth to seek out the clinic and take advantage of the services.

What should the clinic offer and why? How should we scale this up over the years. The programmatic pieces will need to be taken into consideration when developing the physical design concept.

Physical design and construction of the clinic

Working with the organizational architect, develop schematic design; drawing a rough outline of the project, including preliminary room layout, structure and scope within budgetary constraints.
Working with the organizational architect, design and develop detailed architectural plans for approval.
Provide relevant construction documents; converting all aspects of the design into a template from which contractors can estimate costs, identify issues, and plan construction activities.

Operating a clinic

Develop all the relevant standard operating procedures and policies pertaining to the operations of a youth-friendly health clinic.
Build out a three year budget projection.
Develop a system to track medical records/incidents ensuring appropriate patient protection/privacy policies.
Determine and develop a system for the KPIs the organization and clinic need to track to evaluate program/clinic success.

Strategy to gain community buy-in and communicate out.

Avoiding the negativity or stigma that can come with SHR care.
