JOB PURPOSE    
Provide health care services in project supported facilities leading to the achievement of the 95:95:95 targets, especially reducing mother to child transmission of HIV to < 5% and providing care and treatment services to PLHIV. Working with other health workers and project staff to provide quality HIV and MNCH services according to MOH guidelines. KEY TASKS AND RESPONSIBILITIES  Support ANC HIV testing –Target 100% and ART uptake among HIV infected pregnant /Lactating women-Target 100% Ensure viral suppression among PMTCT women -Target >95%
Oversee the provision of Early Infant diagnosis, care and follow-up of HIV-exposed infants –Target 100%
Oversee follow-up and retention of HIV-infected pregnant /lactating women and HIV-exposed infants (HEI) by use of the case management model->95%
Strengthen linkages between PMTCT and HEI services and Comprehensive Care Clinics
Strengthen facility-community /community-facility linkages with the help of CHVs and case managers
Supervise eMTCT services provided by the maternal case managers
Work hand in hand with the clinical officers to implement patients Plan of Care and case management.
Ensure MCH/PMTCT service integration including PrEP and GBV
Perform TB intensified case finding for PLHIV on every clinical encounter-Target 100%
Initiate IPT for all PLHIV who don’t have TB or other Isoniazid contra-indications-Target >90%
Send all PLHIV presenting with presumptive TB for GeneXpert as a first test-Target 100%
Send all patients diagnosed with TB for HIV testing-Target 100% and initiate ART in all newly diagnosed TB/HIV co-infected patients within 2 weeks of starting TB treatment-Target >95%
Ensure that laboratory results are filed, documented and communicated to the patients at all times.
Ensure 12 month /24 month HEI Cohort Analysis (HCA)  ,PCR positive validation and audits and PMTCT reports  is done each month
Participate in MNCH Working Improvement Teams (WITS) and lead team in MNCH in generation, compilation and utilization of data
Strengthen referrals and linkages between PMTCT activities and the Comprehensive Care Clinics and work with OVC partner to ensure clients enrolled and receiving a service
Ensure there is a system in place to track missed appointments and treatment interrupters including use of HIFADHI 
Provide day-to-day technical support of the facility HIV /TB Program as per national guidelines
Provide on job training and mentorship to other health care workers using the national ART guideline
Strengthen linkages between PMTCT, follow-up of HIV-exposed infants (HEI) and Comprehensive Care Clinics
Participate in facility Multidisciplinary team meetings (MDT), represent cases in TWG 
Undertake assessment, planning and implementation of interventions for addressing patient care problems.
Working with other health care providers, promoting the right of patients/clients and maintaining the privacy and dignity of clients/patients and staff.
Prepare and implement annual work plan based on department work plan.
Using your job description and work plan, to continually appraise yourself and rectify any shortcomings or suggest necessary assistance.
Ensure that you undertake requisite professional development activities.
Perform any other official duties that may be assigned by the Facility in charge.

REQUIREMENTS

At least Diploma in Nursing from the Kenya Medical Training College or from a recognized institution.
Registered by the Nursing Council of Kenya.
More than two years’ experience in implementation of PMTCT activities.
At least 2 years of clinical nursing and management experience.
Ability to communicate effectively both orally and in writing and good report writing skills
